Jo is a Licensed Certified Social Worker and an Aging Life Care Professional with 25 years of clinical and managerial experience working with older adults and their families in many different settings. She has worked in rehab and long-term care facilities,
and in positions supporting medical providers who serve older adult patients. She has a great deal of experience in the area of care transitions – providing the information, coordination and advocacy needed to help older adults successfully move from one level of care to another.

Jo served in leadership positions at Springwell, an agency that is part of the State elder services network, including directing the highly regarded Geriatric Care Management Program that has served elders and their caregivers since 2015. With a background in non-profits, Jo has first-hand knowledge and expertise in identifying entitlements/ benefits available through state and federal programs.

As a daughter, Jo navigated the full range of services for both her parents over a 14-year period of increasing needs for support and care. She knows well the joys and challenges of caregiving, and is honored to support others with her knowledge and expertise.

Jo is passionate about listening to each person’s unique story and partnering with them to find ways to meet their needs and improve their quality of life.

Kathy is a Licensed occupational Therapist, a licensed social worker, and an Aging Life Care Professional/Certified Care Manager with Advanced standing. She has over 30 years of professional clinical experience in community, hospital, rehabilitation, and long-term care settings. She has  
expertise in mental health, dementia, and aging in place.

Kathy is recognized as a Certified Care Manager by the National Academy of Certified Care Managers (NACCM), the recognized credentialing organization which assures com- petence to performthe full range of care management tasks.

When working with older adults and the people that care for them, Kathy has a soft touch
and a sense of humor. She doesn't "manage" clients, but creates an alliance, together setting priorities and goals. Each situation is unique and usually involves creative problem-solving to find the optimal solution.
